At your appointment

Planning a visit to an EDS clinic?
Find your clinic’s location and contact details before your visit. You can use this checklist whether you choose Enhanced Dental Studios or another dental clinic.
Explore EDS clinicsHow to prepare your checklist in 5 simple steps
1. Add your appointment details
Write the clinic and location, date, time and contact number. Check the details against your booking confirmation.
2. Check the practical arrangements
Use the prompts to check transport, parking, access, communication needs and your clinic’s booking conditions.
3. Write your most important questions
Start with the question you most want answered. Use the other spaces for anything else you want to discuss; every field is optional.
4. Plan any support and your next step
If someone is joining you, agree how they can help. Keep space for the next step and anything you want to remember after the visit.
5. Print or save your own copy
Choose “Print / save my checklist as PDF”, then your printer or “Save as PDF”. Save before closing or reloading: notes are not kept on this website.

Questions about your appointment checklist.
What is this checklist for?
It is a practical planning worksheet for patients, families and carers. Use it to organise appointment details, write questions and note your next step. It is not medical advice or a way to assess symptoms.
Can I use it with any dental clinic?
Yes. You do not need to book with Enhanced Dental Studios. Follow the instructions your own clinic provides.
Can I edit my answers?
Yes. Change the text or ticks before printing. You can also clear everything using “Clear my notes and ticks”. Closing or reloading the page clears your entries, so print or save first.
Are my notes saved or sent anywhere?
The checklist does not save or send what you type or tick. Notes exist only on this page until you close or reload it. If you print or save a PDF, keep that copy private. The “Copy resource link” button shares only the blank page address.
How do I print it or save a PDF?
Use the print button under the checklist and choose your printer or “Save as PDF” in the print window. For a blank copy, use the two-page PDF download. Long personal notes may add pages.
Can I use it on my phone or with a support person?
Yes. The fields and checkboxes work on a phone. You can complete it together, or print the blank PDF and write by hand. Ask your clinic how to arrange for a support person to join your appointment.
Can our organisation share this resource?
Yes. Link to this page or print unaltered copies for patients, families and carers. It is free to use, with no referral or signup requirement. Enhanced Dental Studios is the creator; this is not an independent clinical guide.
